Requests for service or administrative needs not included in the Complete ADHD Assessment Package - i.e. copies of documents, mailing, phone calls, etc., will be billed separately at an hourly rate of $350/hour.

How to Book an Appointment:
Step 1: Click Book an Appointment and create an account with Profi.
Step 2: Select to schedule either a complementary 20-minute Consultation by phone, or Clinical Interview.
Step 3: Select the Package you prefer and submit payment.
Step 4: Select appointment date and time for Clinical Interview.
Step 5: Complete and submit Intake Forms.
ADHD Assessment Clinic accepts appointments via our HIPPA-compliant online booking platform Profi here, or by phone at 415-891-5889.
Payment Types Accepted:
ADHD Assessment Clinic requires payment in full for each service upon scheduling to confirm an appointment. We do not currently offer payment plan options, nor do we accept insurance.
Payment is accepted via credit or debit card online within the Profi Client dashboard for security purposes. Clients may make payments via mobile or desktop.
Upon receipt of payment, Patient Intake Forms will be sent via email. Forms must be completed and signed, when required, before the first appointment.
Insurance:
ADHD Assessment Clinic is not contracted directly with any insurance provider, given their restrictive limitations on mental health services. Most insurance companies will not cover testing for ADHD, and all do not cover testing for learning disabilities.
To provide services we stand behind and believe are best for our patients, we must therefore operate as an out-of-network provider, and therefore our fees are private pay only.
We are able to provide a paid receipt for services rendered upon request that you may present to your insurance company.
Appointment Management:
Clients may view and manage appointments in the individual Client Dashboard on Profi. Upon following the link to Book an Appointment for the first time, you will be asked to create an account on Profi. Once registered, you can access the Client Login from the link on our homepage, available either via mobile or desktop.
You will receive reminders of upcoming appointments via text, email or phone prior to your appointment.
As our services require a significant investment of time, we kindly request that any changes to a confirmed appointment be made no less than 72 hours or more before a scheduled visit. Notifying as soon as possible allows us to serve other clients on our waitlist. Thank you.
Cancellation Policy:
If you must cancel an appointment, we require 72 hours’ advance notice to receive a full refund of fees paid. Your cancellation must be confirmed as received within 72 hours.
Appointments canceled within 72 hours of the appointment start time incur a fee of $350 plus 20% of service. Your cancellation must be confirmed as received to request a partial refund. Thank you for your understanding.
